INFORMATION & SUPPORT SERVICES

With one phone call to Senior Solutions Information & Support Services, callers can obtain information about agencies, organizations and other resources that can help meet their needs.
Referrals provided to seniors and their families include information about in-home help, locating appropriate senior housing, and information about agencies, organizations and other resources that can help meet their needs. We also educate local companies to refer vulnerable elderly to senior information centers through the Gatekeeper Program.
Support services include in-home needs assessments done by licensed social workers. Social workers help seniors create realistic plans to meet their needs, and assist in putting resources and services in place. Intensive case management, care coordination, consultations, education and specialized assistance are also available through our Eldercare Services program, on a for-pay basis, to help people with the challenges of caring for elderly family members.

ELDERCARE SERVICES

This program provides specialized assistance to people who have particular challenges caring for elderly family members. Our staff, which includes a Professional Geriatric Care Manager and licensed social workers, provides gentle and dignified care for the elderly.
Services include assessments, care plans and management, guardianship/conservatorship education, counseling, advocacy, consultations, information and referrals and crisis intervention. There is an hourly fee for this service.
In cases where there is no family or a senior does not want family involved, Senior Solutions offers Power-of-Attorney services. Seniors are encouraged to plan ahead and think about who they choose to make decisions about their health, living arrangements and well-being when they are no longer able to make decisions for themselves.
