Hospice is rooted in the centuries-old idea of offering hospitality to those on a long journey. It is about living life as fully as possible because each of us matters to the last moment of our lives. That was the essence of Dame Cicely Saunders beliefs as she founded modern hospice care.
The focus of hospice is on the improvement of the quality of life. Hospice services support you in giving those you love the best you can offer in end-of life care. Hospice recognizes death as the final stage of life. We enable hospice patients to live their final days to the fullest surrounded by their loved ones.

Horizon Hospice provides comprehensive and practical end-of-life care that supports both the patient and the family. Our emphasis is on protecting the dignity of the patient while offering the best comfort care available. We offer a holistic approach in the services we provide.
Care is provided by a team of qualified caring professionals
- Routine visits in your home by nurse case managers to provide pain control and symptom management. A nurse is always available on-call for crisis management.
- Nurse’s aides are available to provide personal care and family support.
- Horizon Hospice makes every effort to treat body, mind and spirit as a whole. To this end, we are staffed to provide you with spiritual services that are non-denominational and non-judgmental. We also help connect families with their choice of spiritual support.
- Our social workers are skilled in a number of areas that include securing community resources, assisting in financial issues, facilitating completion of advanced directives, and providing initial counseling.
- Bereavement support is offered to support families while their loved one is on hospice care and for 13 months following the death. We also provide education and ongoing bereavement support on a community wide basis.
- Volunteers offer respite care, patient companionship and are also available for errands, transportation assistance, and other care. (for volunteer information, click here.)
In addition to our interdisciplinary team of professionals and volunteers, Horizon Hospice also supplies medications for pain and symptom management related to the terminal illness, and medical equipment needed for comfort in the home setting.
Medical Social Work
Home Health
Home Health Social Workers assess the biological, emotional, and social factors related to the patient’s illness and how these factors affect the progress or stability of the patient’s medical condition. Following this assessment, the social worker implements a plan of care to obtain available community and financial resources to assist the patient in resolving issues. The social worker may also provide short-term counseling services to the patient for adjustment to illness, to intervene with family members or other caregivers to resolve conflicts or remove barriers to effective treatment. The primary goal is to keep the patient safely in his/her home with sufficient support/services. Medical Social Services must be ordered by the physician and is included in the plan of treatment. The services must be provided by a licensed social worker.

Bereavement
The Horizon Hospice bereavement program consists primarily of a thirteen month post death follow-up for the families of our patients. This may include phone calls, visits, and mailings. The frequency and types of contacts for the families are tailored to meet their individual needs. Through our bereavement program families have access to hand-outs, bibliographies, books, and videos that address their specific bereavement concerns.
In addition to the services for the families of our patients, we offer a group on the third Thursday of each month that is open to the public. Grief classes for specific groups and topics are available at different times throughout the year. We are also happy to make referrals to other bereavement services and to private counselors as needed.
